Job Responsibilities
· Provide primary support and supervision to youth
· Organize, facilitate and participate in recreational activities with youth
· Assist youth with daily living, social, work readiness and recreational life skills
· Complete daily programmatic paperwork, including treatment service notes
· Attend mandatory trainings
· Utilize and implement behavior support techniques
· Model healthy boundaries and social skills within the program milieu
· Provide a safe and nurturing environment for youth
· Verbal and written communication with all members of the Deaconess team
Schedule
Shift 1:
· Sunday: 5:00 pm to 10:00pm
· Monday: 8:00 am to 7:00 pm
· Tuesday: 8:00 am to 10:00 pm
· Wednesday: 9:00 am to 12:00 pm then return 3:00 pm to 10:00 pm
OR
Shift 2:
· Wednesday 9:00 am – 12:00 pm
· Thursday: 8:00 am to 9:00 pm
· Friday: 9:00 am to 9:00 pm
· Saturday: 9:00 am to 9:00 pm
Qualifications
· High School Diploma or equivalent required.
· Experience in relevant organization.
· Valid driver’s license.
· Proven oral and written communication skills.
· Mature, assertive, and organized.
· Ability to multi-task.

The Director of Data and Compliance leads agency-wide data management, accreditation, and quality improvement efforts to support the effectiveness of services at the Deaconess Home. This role oversees Council on Accreditation (COA) compliance and Performance and Quality Improvement (PQI) initiatives, ensuring programs measure outcomes, meet standards, and use data to drive continuous improvement.
Job Responsibilities
· Lead agency accreditation and compliance efforts, including COA and mandated service data reporting
· Serve as Chair of the Performance and Quality Improvement (PQI) Committee
· Facilitate PQI meetings and oversee implementation of the PQI Plan
· Analyze program data, identify trends, and develop quality improvement initiatives
· Develop and manage Risk Management and Quality Improvement Plans and reports
· Oversee chart reviews and electronic records systems
· Develop and maintain agency databases for program, administrative, employee, and client data
· Ensure data accuracy, integrity, and confidentiality
· Create monthly reports for the Leadership Team
· Train staff on database use, data entry standards, and confidentiality requirements
· Support agency-wide strategic planning and quality improvement efforts
· Assist with staff, youth, and stakeholder surveys
· Attend required meetings and trainings
· Perform additional duties as assigned
Qualifications
· Master’s degree preferred
· Strongly preferred: experience in compliance, accreditation, and PQI within a human services or residential youth-serving organization
· Strong analytical, written, and verbal communication skills
· Demonstrated ability to collect, analyze, and interpret data to support quality improvement
· Ability to work collaboratively with staff, youth, families, and external partners
· High level of organization, adaptability, and professionalism

The Fall River Deaconess Home is currently seeking a Licensed Practical Nurse to join our residential healthcare team. This is a part-time position that includes full benefits. Recent graduates are welcome!
The LPN reports directly to the Nurse Manager and is a member of the overall agency team providing direct services to youth and their families.
Job Responsibilities
· Medication administration
· Triage of medical needs of the youth in the program
· Assistance with health record maintenance
· Direct resident supervision
· Utilize and implement behavior support techniques
· Reporting on youth quarterly progress towards health care goals
· Scheduling and attending health care appointments
· Review immunization records, order and administer necessary immunization
· Health care education to youth
· Participate in intakes of youth to the program
· Monitor weight and nutrition need status of youth
· Participate in the on-call rotation
Schedule
· Wednesday all-staff agency meeting from 8:15am – 10:00am
Qualifications
· Licensed Practical Nursing license from board approved practical nursing program
· Proven oral and written communication skills
· Significant attention to detail, and the ability to work collegially with a wide variety of individuals inside and outside the program

The Fall River Deaconess Home is committed to both the youth and staff of our program. We are currently seeking an Occupational Therapist to work in a therapeutic environment and join our treatment team.
The Occupational Therapist reports directly to the Director of Clinical Services and is a member of the agency team, responsible for planning and delivering OT services to youth in a residential setting.
Job Responsibilities
· Provide direct Occupational Therapy evaluations and re-evaluations, including assessment of prior and current level of function.
· Setting individualized goals, assisting program staff to implement within the milieu and working with youth and family to transfer treatment in the home
· Provide direct OT treatments according to standards of practice, working with agency staff, youth and family regarding the progress of treatment planning goals
· Report on child’s progress through appropriate written documentation including evaluation forms, progress notes, education forms.
· Collaborate with the agency’s health services, clinical and milieu team, children, family, and other members of the youth’s treatment team to develop treatment plans goals and implement services.
· Attend Treatment and agency meetings
Schedule
· Wednesday all-staff agency meeting from 8:15am – 10:00am
Qualifications
· Initial certification by National Board for Certification of Occupational Therapy
· Current licensing by the American Occupational Therapy Certification Board
· Registered with the State Board of Registration in Allied Health Professionals
· Two years of experience as an occupational therapist, preferably in school system or other pediatric practice setting
· Pass all necessary background checks.
