Job Responsibilities

· Wednesday all-staff agency meeting from 8:15am – 10:00am

Summary
The Fall River Deaconess Home is committed to both the youth and families within our communities. We are currently seeking a Clinician to work in a supportive environment and join our treatment team in providing quality and comprehensive services to youth and families in their homes, schools and communities. This is a full time, 40 hours a week position.

Job Responsibilities
· Provide clinical consulting and training as needed to the Support & Stabilization team
· Conduct in-home individual and family therapy
· Provides direct clinical assessment, psycho-therapeutic treatment, and crisis intervention to clients and their families
· Complete case documentation and other administrative responsibilities to meet agency and licensing standards
· Collaborate with the team and external service providers to coordinate effective client services
· Participate in scheduled multidisciplinary treatment team meetings and utilization reviews
· Facilitate clinical groups
· Assist with developing, implementing, evaluating, and revising overall treatment goals and plans in conjunction with the team
· Conducts assessments to include but not limited to CANS, CBT, Virtual Gateway, Trauma-Informed Care of psychiatric history and comprehensive assessment and safety planning for all risk issues.
· Responsible for entering information into Virtual Gateway
· Demonstrates the knowledge and skills necessary to provide care to a culturally diverse patient population

Qualifications
· Master’s level education in Social Work, Psychology, Counseling, Human Services or a related field (required)
· Valid Driver’s License

This part-time role is vital in providing compassionate care to our residents while supporting our mission of enhancing the well-being of youth and families through quality healthcare services.

Job Responsibilities
· Provide direct patient care under the supervision of registered nurses.
· Administer medications and treatments as prescribed.
· Monitor patient vital signs and report changes to the healthcare team.
· Assist with daily living activities for residents.
· Maintain accurate medical records and documentation.
· Collaborate with multidisciplinary teams to develop patient care plans.
· Educate youth and families on health management strategies.
· Ensure compliance with health regulations and standards.

Wednesday: 9am – 2pm
Thursday: 5pm – 10pm

Qualifications
· Valid LPN license in the state of Massachusetts.

Summary
The Program Manager provides supervision and coordination of all activities to achieve consistent overall program implementation in a Group Home setting: scheduling, supervision and evaluation of direct care staff, participation in recreation planning, developmental and review; implementation of all techniques, strategies, and procedures to protect the health and safety of all clients.

Job Responsibilities
· Provide primary support and supervision to youth
· Responsible for balancing program petty cash and youth allowances
· Provide support, supervision, and guidance for staff
· Participate in on-call responsibilities on a rotating basis
· Will perform daily responsibilities within the milieus alongside supervisors and Direct Care Staff
· Responsible for delegating programmatic responsibilities to Direct Care Staff
· Completing and responsible for ensuring all necessary paperwork
· Organize, facilitate, and participate in recreational, life, and social skills activities with youth
· Attend mandatory trainings
· Utilize and implement behavior support techniques
· Model healthy boundaries and social skills within the program milieu for youth and staff
· Provide a safe and nurturing living and working environment for youth and employees

Schedule: Five consecutive days, 2nd Shift

Qualifications
· Bachelor’s degree preferred
· 5 years of experience in Social Service or 2 years supervisory experience in related field
· Candidates must be 21 years of age or older
· Valid Driver’s License
· Proof of vehicle insurance
· Pass all necessary background checks
