Job Responsibilities
· Monitors and documents residents’ sleep schedules and individual needs
· Support residents in daily living activities and personal care
· Administer medication if MAP certified.
· Maintains program routines and documentation
· Completes weekly schedule of both routine and non-routine activities

· Collaborate with treatment team
· Accompanies residents in the community

We are seeking a Case Manager to join our Support and Stabilization team located in Cape Cod, MA. The Case Manager provides comprehensive community-based services to an assigned caseload of youth and families within the program. This role delivers supportive services in homes, schools, and community settings; coordinates access to basic needs and community resources; and collaborates with service providers, funding agencies, and treatment teams to support youth stability and growth. The Case Manager documents services and treatment activity, maintains required records, and promotes trauma-responsive, and strength-based practices.
Job Responsibilities
· Manages an assigned caseload, working collaboratively with youth, families, funding agencies, and treatment teams to identify and address the priority needs of each youth and family
· Provide supportive services within youths’ homes, schools, and community settings including transportation as needed
· Provide outreach and community-based mentoring which may focus on teaching personal skills related to self-care, social skills, education and pre-vocational attainment and engaging youth in age appropriate social and recreational activities
· Maintain record keeping requirements in accordance with agency standards and contractual obligations
· Coordinates access to basic needs, including but not limited to food, shelter, and utilities while identifying community-based resources, managing referrals, engagement, follow-up and service coordination
· Attend and participate in supervision, staff meetings, trainings, conferences and workshops to promote professional development
· Builds positive, collaborative relationships with youth and families while delivering strength-based services using a wraparound approach
· Utilizes a digital case management system to accurately document daily events and engagement with youth, families and collateral contacts
· Utilizes the digital case management system to develop treatment plans, progress reviews incident reports and discharge summaries
· Maintain ongoing collaboration with other service providers and funding agencies
· Participate in appropriate clinical and administrative staff meetings and in-service trainings
· Travel within program catchment area to deliver and coordinate services

The Fall River Deaconess Home is committed to both the youth and staff of our program. We are currently seeking an Occupational Therapist to work in a therapeutic environment and join our treatment team.
The Occupational Therapist reports directly to the Director of Clinical Services and is a member of the agency team, responsible for planning and delivering OT services to youth in a residential setting.
Job Responsibilities
· Provide direct Occupational Therapy evaluations and re-evaluations, including assessment of prior and current level of function.
· Setting individualized goals, assisting program staff to implement within the milieu and working with youth and family to transfer treatment in the home
· Provide direct OT treatments according to standards of practice, working with agency staff, youth and family regarding the progress of treatment planning goals
· Report on child’s progress through appropriate written documentation including evaluation forms, progress notes, education forms.
· Collaborate with the agency’s health services, clinical and milieu team, children, family, and other members of the youth’s treatment team to develop treatment plans goals and implement services.
